My chain came off and having trouble reinstalling There...

My chain came off and having trouble reinstalling. There are two holes for the slide mechanism and I have one screw (part #33) m4 x 10. Should there be two of these screws or just the one? Directions or photo on how to reinstall the chain would be most helpful.
